For your first visit, you will need to arrive 10 minutes early to complete the necessary paperwork. These forms include:
New Patient Information - includes patient information, insurance, phone numbers, accident information, patient condition, health history, and medications/vitamins.
HIPPA Notice of Privacy Practices - describes how your protected health information may be used and disclosed by Dr. Hoefert, our office staff, and others outside of our office that are involved in your care and treatment for the purpose of providing health care services to you, to pay your health care bills, to support the operation of Dr. Hoefert's practice, and any other use required by law.
Financial Policy - states that we will submit all your claims to any insurance you may have and will file the necessary paperwork, but ultimately you are responsible for your account balance remaining with Lyon County Chiropractic.
Terms of Acceptance - explains what an adjustment is and the risks associated with any medical treatment, including chiropractic. By signing the Terms of Acceptance, you give Dr. Hoefert permission to treat you.
Other forms may include a Low Back or Neck Questionnaire or a Vehicle Accident Information form, depending on your area of complaint.
Dr. Hoefert will spend approximately 30 minutes with you during your first visit. He will complete a detailed health history of your condition and perform a thorough examination. The new patient examination may include x-rays, but only if medically necessary. If Dr. Hoefert determines that chiropractic care is appropriate, you will receive a treatment during this initial visit. If Dr. Hoefert determines a medical referral is necessary, he will make his recommendations during your first visit.
After the treatment, Dr. Hoefert will take time to explain your condition to you and recommend exercises you can do at home to aid with recovery as well as to prevent a recurrence. The goals for your treatment will also be set at this time. Dr. Hoefert welcomes any questions or concerns you may have throughout the course of your chiropractic visit, so please feel free to discuss your health concerns with Dr. Hoefert at any time. We will close the first visit by determining a course of care that is appropriate for your specific condition.
